Condition of Bicycle / Distinguishing Features: 
In very good condition, only a year old. Frame is stock white with all other parts, handle bars, rims, seat, accessories black. Had a Bell bottle holder on the bottom angle frame tube. A mount was also attached to the vertical frame tube that holds the seat pole in it for a lick, plastic. Two mounts on the handle bars, one was large and stuck out from the front of the handle bars for a large Nashbar handle bar bag. The other was much smaller, to the left of the the neck of the handle bars for a bike light.

Incident Description: 
Went into CVS for a Minute Clinic appointment. Locked bike next to parking meter box, to a bike rack steel U pole bolted to ground. Wrapped lock through front and rear wheels and frame to pole. Removed all accessories except for mounts described above. Appointment lasted for about an hour and a half. After appointment was over, came to discover bike was gone and no evidence of lock was found. Asked CVS and Jewel Osco (directly across from the CVS on Southport) if they have cameras pointing out to the area I parked my bike and they said they do not have cameras pointing in that direction.
